1. Referral and Initial Assessment of Young People and Referral
Assessment of young people by caseholder at YOS using ASSET
If score is 2+ on substance misuse section then the young person will be referred to the substance misuse worker
The substance misuse worker will meet with the young person within 5 working days for Initial Assessment.
Depending on the outcome of the assessment, a second appointment will be made for a tier 2 service education and awareness session, plus an information pack with details of local services for young people will be given
If the substance misuse worker’s Initial Assessment identifies that a further intervention is required then a second appointment will be made within 10 working days for an in-depth assessment
Subsequent appointments will be made with the young person on a weekly or fortnightly basis in agreement with the supervising officer, young person and substance misuse worker, taking into account the young person’s commitments (i.e. school, college, work etc.).
The expected duration of appointments would not exceed 12 sessions unless exceptional circumstances prevailed.
Ongoing 1:1 key-working sessions will be based on developing a therapeutic relationship with the young person to be able at gain a holistic view of the young person’s circumstances.
2. Allocations
All new referrals that have been initially assessed are presented at allocations meetings held on Thursdays at Sorted. The aims will be to identify the young people’s needs, assess the level of intervention required and allocate to the appropriate key worker.
Depending on the results of the Initial Assessment the substance misuse worker will identify the level of intervention required
The young person’s supervising officer will carry out all Tier 1 interventions
Tier 2 interventions will offer the young person a comprehensive education and awareness session, usually lasting up to 1 hour to include advice and information regarding young people’s services in the borough.
A further, more in-depth assessment will be carried out by the substance misuse worker if there are further issues identified that may require ongoing 1:1 key working or a Tier 3 or 4 interventions
If a young person was assessed as needing a Tier 3 or 4 intervention, a referral to Sorted would be advised with the young person’s consent. A record of attendance would be required by YOS, as all appointments with the substance misuse worker are statutory and recording on Careworks is mandatory. The case-holder will also be informed of any missed appointments which may contribute towards breach proceedings being issued. All discussions and content of the sessions would remain confidential between the young person and the substance misuse worker and would not be disclosed to any other persons.
All young people will be recorded on the New Clients Initial Contact Form as a legal requirement of the NDTMS.
Also, a Sorted YPD&AT-Supplementary Intake Sheet will be completed by the substance misuse worker and a file will be opened for the young person, which will remain at Sorted.
Clients’ details will be recorded in the Sorted ‘Referrals and allocations log’.
Closure forms will be completed by the substance misuse worker when an intervention has been successfully completed OR the young person’s intervention plan has been revoked for non-attendance/compliance of order.
3. Evaluation
A Tier 2 evaluation form will be given to young people at final intervention session, by the substance misuse worker, for monitoring purposes only. The identity of young people will remain confidential
